Typically, medication is the first treatment recommended to those with erectile dysfunction. Many men experience relief of symptoms after using medication. Our doctors use the results of diagnostic tests to determine which medication is best suited for you. The first treatment usually offered to men with erectile dysfunction is a prescription medication, such as sildenafil (Viagra®), tadalafil (Cialis®), or vardenafil (Levitra®). Generally taken 30 to 60 minutes before sexual activity, these fildena super active 150mg medications work by improving blood flow to the penis during arousal.

Erectile dysfunction pills have some differences, but price can limit your ED medicine choices.

“See a urologist who has experience in penile implant surgery to discuss your options.” Steer clear of herbal medications and “natural supplements” for ED. No studies have proven that they work, and they could be dangerous. “The FDA doesn’t regulate ED supplements, so you don’t know what you’re getting,” says Dr. Liroff. “At best, you’re paying for something that won’t give you results.

And at worst, you could experience dangerous interactions and side effects.” One of the newest therapies for ED is a gel you apply directly to the penis. This treatment is not yet sold in the U.S. “We need more clinical trials to show that an ED topical gel is as effective as other options,” says Dr. Liroff. “Pills, injections, pumps and shockwave therapy all produce good results and have evidence to show that they work.” Platelet-rich plasma (PRP) and stem cell therapy are two of the newest potential therapies for ED. Erectile dysfunction medications like Viagra® should not be used by men who take nitrate medications to treat the symptoms of heart disease. Erectile dysfunction medications can cause a drop in blood pressure. When combined with nitrates, which dilate or open up the arteries to the heart to increase blood flow, there is the potential for a severe drop in blood pressure.

Some 35 percent of men do not respond to oral medications for erectile dysfunction. A smaller percentage of men may experience uncomfortable side effects, such as flushing or feeling suddenly warm, changes in vision, upset stomach, or heartburn. If, after several months, you have not seen an improvement in these side effects, your doctor may offer other medical treatment options. Intraurethral medication is often used as an alternative in men who don’t respond well to oral medications for erectile dysfunction. This is a suppository inserted through the opening in the penis. Using a narrow, prefilled plastic applicator, the man inserts the medicine directly into the urethra 5 to 10 minutes prior to sexual intercourse. The medication causes the muscles of the penis to relax and the arteries to widen, enabling adequate blood flow for an erection.

Increase the Dose

However, there’s no solid evidence of their effectiveness. “Right now, PRP and stem cell therapy are unproven,” says Dr. Liroff. “Some small studies say they could help with ED, but others show no results. There are also a lot of questions about how much is effective and which patients might benefit.

Because some men experience dizziness and, rarely, fainting, within an hour of using these medications, your doctor may recommend that you refrain from driving or operating heavy machinery. Penile self-injections were the first medication approved by the U.S. Food and Drug Administration for the treatment of erectile dysfunction. These are prescribed for men who don’t respond well to oral medications. An NYU Langone urologist performs the first injection in the doctor’s office, in order to determine the dose of medication needed. Your doctor then instructs you on how to safely inject the medication, which is supplied in vials with syringes, for use at home. Typically, a man injects the medication into the shaft of the penis 10 to 20 minutes before sexual activity to achieve an erection. Side effects can include an aching sensation in the penis, which generally goes away in a few hours.

“LISWT causes mild tingling, but most patients don’t find it painful,” says Dr. Liroff. “When a licensed provider gives you shockwave therapy, it’s safe and has a minimal risk of side effects.” It’s important to know the difference between true shockwave therapy and counterfeit versions. A similar-sounding treatment known as radial wave therapy (rWT) is available at healthcare clinics and even non-medical facilities. Though it promises results, it has no science to back up its claims.

Which Medication Will Work Best for My ED?

“Shockwave therapy has data to show that it stimulates blood flow and generates new blood vessels,” explains Dr. Liroff. “Radial wave therapy doesn’t have these results, and studies have shown that it’s equal to no treatment.” Penile prosthesis surgery, sometimes called a penile implant, might be an option if less invasive treatments haven’t worked for you. “There are different types of implants, depending on your needs and preferences,” says Dr. Liroff. Patients need sexual stimulation and foreplay to release NO initiate an erection in order for these medications to be effective. This class of medication is a safe and effective oral treatment for men with erectile dysfunction of physical, psychological or mixed cause. However, it is important that patients should be aware of the low probability (less than 50%) that intercourse will be possible after the first dose – particularly in severe or advanced cases and especially when using Viagra or Levitra. The majority of men who stop Viagra because of apparent lack of effect will in fact respond and achieve intercourse if they continue to try again, progress from 50 mg to 100 mg, or take the pill without food on an empty stomach (3 hours after eating). Taking Viagra or Levitra on a full stomach will not only reduce their peak concentration in the body by 30 to 50% but in addition will delay their peak concentration by one hour. In other words one may have to wait after intake before initiating sexual activity. Cialis peak concentration and time before reaching maximum absorption is not affected by a full stomach. Because of this it remains my drug of choice for the treatment of ED and I always prescribe it first. In addition many of my patients experience fewer side effects with Cialis then with the other two. Finally Cialis has a long period of activity and patients are successful at intercourse up to 36 hours after taking this pill. This represents a distinct advantage from the patient’s perspective as it diminishes the need for planning. Sex can be more spontaneous and natural.

Reliable safety data gathered in the 10 years since these PDE % inhibitors were launched confirm that side effects such as Priapism (prolonged erection), red eyes, painful eyes, syncope(fainting), tachycardia (rapid heart rate), and nausea do occur, but are rare.
